FOR TOURISM
- Duly perfected visa application form (Black or Blue ink)
- Passport valid for not less than six (6) months after sojourn in the Philippines and photocopies of the bio metric and the alteration page
- Bank Statements of the applicant with substantial transactions for the past six (6) months.
- Submit the original statements along with a set of copies, If the copies/E-statements are presented, to be certified by the bank.
- The updated bank book along with the copies for the last six (6) months. (If the statements are not available)
- Minimum balance of Rs. 300,000 or more with continuous maintenance, Fixed Deposits can be shown as an optional document, deposited 3 months before. (Bank statements from foreign countries are not accepted)
For incentive tours when the sponsorship is by the company, the letter to be signed by Two Directors or One Director & a Senior Manager, along with the Form 20. Tickets or flight reservation (entry and exit) Hotel accommodation or proof of lodging Proof of financial capacity (any of the following)
- Bank statement of the latest six (6) months
- Employment certificate with remuneration
- Proof of Business income
- Letter of guarantee from the sponsor with proof of financial capacity (3 months bank statements certified by the bank in Philippines)
If sponsored, a Letter of invitation (duly authenticated by a notary public in Philippines), sponsor’s passport copy Copy of the Business Registration Certificate, Form 20, certified by company secretary for company sponsorships. A letter from the employer granting leave for the period of stay and the confirmation of returning to Sri Lanka. Documents proving ties to the home country/ country of residence (any of the following)
- Employment certificate
- Property documents
RT-PCR and antigen tests are no longer required prior to departure for passengers 18 years old and above who have been fully-vaccinated and have received their booster shots;
- RT-PCR and antigen tests are no longer required prior to departure for fully vaccinated passengers 12-17 years old;
- Passengers 12 years old and below are exempted if the accompanying adult is fully vaccinated and have received their booster shot;
- Travel insurance is no longer required; and
- Filling out of the One Health Pass prior to departure is still in place.
Two copies of 2″ x 2″ recent coloured photographs A police clearance may be required later by the Philippine Consulate Visa fees: Single Entry valid for three (3) months- US $ 50 (Fees to be paid in LKR subject to the current exchange rate) Processing period: 10 days
- Group consisting of ten (10) or more applicants, the applications to be submitted before 11.00 am
ADDITIONAL REQUIREMENTS FOR SPECIAL CASES: For Minor applicants:
- Barangay Certificate of the Filipino spouse, children, or parent (Period of validity-6 months)-If Filipino Spouse /Parent is not travelling with applicant
- Birth certificate – Original authenticated by Philippine Statistics authority and a copy /Report of Birth
- Marriage certificate of parents- Original authenticated by Philippine Statistics authority and a copy/ Report of Marriage
- Identification of the inviting Filipino parent.
- Copy of parents’ passports
- Passport and Photocopy of its data page, previous visa issued and the latest Philippine Immigration stamps.
- The Letter from the parents confirming that the child is accompanied by them
- In the event of a parent not accompanying the child/children, parent/guardian must submit a letter of consent from parents
- For Students: letter from the school granting permission to travel overseas.
For applicants married to a Filipino (Spouse):
- Barangay Certificate of the Filipino spouse, children, (Period of validity-6 months)- If Filipino Spouse /Parent is not travelling with applicant
- Invitation letter from the spouse and Notarized by Notary Public in Republic of Philippines.
- • Marriage certificate -Original authenticated by Philippine Statistics authority and a copy/ Report of Marriage
- Identification of the inviting Filipino spouse, Children.
- Passport and Photocopy of its data page, previous visa issued and the latest Philippine Immigration stamps.
- Copy of spouse’s passport
FOR SEAMAN
- Duly perfected visa application form (Black or Blue ink)
- Photocopies of the passport bio metric, alteration page and the Seaman’s Book,
- Passport valid for not less than six (6) months after sojourn in the Philippines
- Letter from the Local and the Foreign agents addressed to the Consulate
- Two copies of 2″ x 2″ recent coloured photographs
- Letter from Ministry of Ports and Shipping
- Travel Itinerary
- Proof of financial capability
- Negative RT-PCR test, taken within forty-eight (48) hours; or a negative laboratory-based antigen test taken within twenty-four (24) hours, prior to the time of departure from the country of origin/first port of embarkation.
- Travel insurance, for COVID-19 treatment costs, with a minimum coverage of USD 35,000 for the duration of stay in the Philippines.
- Acceptable proofs of vaccination, which includes the following: – World Health Organization International Certificate of Vaccination and Prophylaxis, or – VaxCertPH,or – National or state digital certificate of the country/foreign government, which has accepted VaxCertPH under a reciprocal agreement
- Visa fee US $ 30 (Fees to be paid in LKR subject to the current exchange rate) Processing period: 07 days
